The Consent and Liability Waiver Forms provided on this web site can be printed, completed (with parent or guardian signature) and given to Pam or mailed to her at:
- Student Registration Form
Are you a new high school teen who is interested in trying out Youth Ministry? Fill out this form and bring it with you to your first meeting (see the schedule for more information). New teens are always welcome!
- Parent Registration Form
If you are a parent and have an interest in volunteering your talents to help create a great Youth Ministry program, please fill out this form and let Pam know.
- Activity Consent Form
The Activity Consent Form must be signed and turned in for any event where we are required to leave church property (field trips, etc)
